Resolution/Fix/Answer

Sometimes the Trigger Batch Form in AP04 - Create New Batch Form Queue is locked.  This can be overridden via 'Override Auto-Feed' Button within Unity Client which will allow the 'New Batch Form' Button to display due to the Trigger Document showing up.
